Zanmi Lasante’s fieldwork in the Plateau-Central region is structured around a clear objective: to guarantee continuity of healthcare services and social support through close coordination between local teams, healthcare structures and community players.

Multidisciplinary teams operate at several network sites, working directly with medical departments, social workers and community teams. This work is based on regular exchanges, the review of follow-up practices and the organization of interventions at local level. It ensures consistency between clinical services, community follow-up and the social support provided to beneficiaries.

Field work also involves travel to areas where access to communities is not based on conventional infrastructures. Teams use alternative routes to reach health centers and monitoring areas. These journeys are not peripheral to the work. They condition the ability to maintain direct contact with beneficiaries, to understand local realities and to ensure regular follow-up.

Meetings with teams on site enable us to analyze how beneficiaries are monitored, to identify any adjustments needed and to strengthen coordination between the various players involved. These exchanges focus on concrete elements: organization of community visits, circulation of information, data management, articulation between social services and health care.

At the La Colline Health Center in Lascahobas, an elderly beneficiary shared his experience of the follow-up he receives. His testimony highlights the importance of the support received in his daily life:

The interventions supported by the IDB and FAES combine social support, community monitoring and coordination with health structures. Local teams ensure the identification of beneficiaries, support and continuity of services, in constant liaison with field teams.

This work relies on a regular presence in communities, organization adapted to access constraints and ongoing coordination between the players involved. It is part of an approach in which the monitoring of beneficiaries and the quality of services depend directly on proximity to the field and the collective work of local teams.
